Supports Coordination

In New Jersey, individuals with intellectual and/or developmental disabilities use a support coordinator to complete the Person-Centered Planning Tool (PCPT) and create the Individualized Service Plan (ISP). Support coordinators work for DDD/Medicaid-approved Support Coordination Agencies and are required to make monthly contact with each person for health and safety monitoring.

Individuals and their families can choose a support coordination agency or choose to be auto assigned by DDD. You may change your Support Coordination Agency at any time by completing a DDD online form. Your support coordinator can assist you in planning for a successful live in the community of your choice.
