Transition to Adult Services
Transition planning to adult life should begin at age 14 with the person’s school district. When an individual with intellectual and/or developmental disabilities completes their educational entitlement at age 21, they may become eligible for supports and services from The Division of Developmental Disabilities (DDD). DDD eligibility is determined by completing an application to the Division, which can be submitted before a person turns 21 years of age. In addition, an individual must be Medicaid-eligible and have received at least one Supplemental Security Income payment.
